A summary care record (SCR) is an electronic summary of key clinical information about a patient (including medicines, allergies and adverse reactions), sourced from the GP record. It is used by authorised healthcare professional, with the patient's consent to support their care and treatment.

National Data Opt-out

NHS England have launched the ‘National Data Opt-out’, this is a new service that allows people to opt out of their confidential patient information being used for research and planning. This is provided in line with the recommendations of the National Data Guardian in her Review of Data Security, Consent and Opt-Outs and replaces the 'Summary Care Record.

Patients can view or change their national data opt-out choice at any time by using the online service at www.nhs.uk/your-nhs-data-matters or by calling 0300 3035678.

The East London Health & Care Partnership has stated that the quality of care you receive can be improved by sharing the right information about you between local NHS and social care organisations who are providing you with your direct care.
