We are happy to register new patients who have moved into our practice area. We do not discrimate on the basis of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical conditions.
Our practice boundary is broadly covered as follows:
- NORTH: Seymour Road N8
- EAST: Stoke Newington Road N16, Stoke Newington High Street N16, Stamford Hill N16
- SOUTH: Matthias Road N16, Pellerin Road N16
- WEST:Blackstock Road (up to no.156) N4, Seven Sisters Road (nos.269-523) N4, Wightman Road (up to no.149) N4
To check whether a specific address is within our catchment area or for further information about our registration procedure please contact our reception team.
If you wish to come into the practice in person to register you may do so during our opening hours, Monday - Friday between 8:00am and 6:30pm.
You will need to bring proof of address with you in order to register. This can be any one of the following documents and must be dated within the last 4 weeks:
- Bank statement
- Tenancy agreement
- Gas or electricity bill
- Telephone bill (not a mobile)
- Council Tax bill
- Photo ID Driving Licence
When your application has been processed we will ask you to come for a New Patient Health Check appointment. This is so we can record information about your medical history and health requirements. Please bring any information about medicines and children's vaccinations to this appointment.
Pre-Registration with the practice
If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct and to provide proof of address.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice with your proof of address.
