As you may be aware the Heron Practice is a teaching practice.
We are pleased to welcome Dr Amy Short who will be starting a twelve month "in service" placement with us on 10th August.

Telephone Consultations
Patients who want to discuss health problems with a doctor and feel they don't need to be examined can request a telephone consultation; these can be booked up to 1 week in advance. 
Please note these appointments are NOT for general information/advice that can be obtained via NHS Choices or NHS Direct.

Patient Registration

We are happy to register new patients who have moved into our practice area. We do not discrimate on the basis of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical conditions.

When your application has been processed we will ask you to come for a New Patient Health Check appointment. This is so we can record information about your medical history and health requirements. Please bring any information about medicines and children's vaccinations to this appointment.

Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this form too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
